Box 26 - Patient's Account No.

Box26 provides the option of including a Patient Account Number if required.  Whether or not this information appears is determine on an insurance carrier by carrier basis.

• The Patient Account Number (called a 'User Code' in DELPHI) is entered in the Client Information Screen.
• The Insurance Carrier Setup screen allows you to choose if the 'User Code' is printed in Box26 for each individual insurance carrier.
