Box 4 - Insured's Name (Last, First, Middle Initial)

Box 4 contains the name of the carrier of the policy under which the client is covered.  If the client is not the carrier of their own policy,  then they are most likely the covered by a policy held by their spouse, parent, or significant other.  This information is entered at the bottom of the Client Insurance Information Screen in the section labeled "Carrier/Owner of the Insurance Policy".
