Box 9d - Insurance Plan Name or Program Name

Boxes 9, 9a, 9b, 9c, and 9d contain information related to secondary insurance coverage if it exists.

Box 9d contains the Insurance Plan Name or Program Name of the secondary insurance policy under which the client is covered.  This is simply the name of their secondary insurance carrier if it exists.  This information is determined in the Client Insurance Information. Screen based on the ranking of the primary versus. secondary checkboxes located on that screen.
