Box 13 - Authorize to pay benefits to supplier of service

Box 13 is a signature box on the HCFA-1500 that contains the Authorization to pay benefits to supplier of service..  This information is entered in the Client Medical Information screen.

The Client Medical Information screen contains a checkbox for "Authorize Payments."  When this item is checked, DELPHI32 will print "SIGNATURE ON FILE" in Box13.
