Box 2 - Patient's Name (Last, First, Middle Initial)

Box 2 contains the Patient's Name which is entered in the Client Information Screen.  The client information screen breaks down the name into First, Middle Initial, and Last Name.  These items are rearranged to Last, First, Middle Initial for this box.
