Box 21 - Diagnosis or Nature of Illness or Injury

Box 21 contains the diagnostic codes related to this patient.  This information is entered in the Client Medical Information screen.  DELPHI32 allows for up to 4 diagnosis codes which correspond to the HCFA-1500.

Although entering the diagnostic codes for the client is fairly straightforward, there are several additional considerations related to diagnostic codes.  

The basics about diagnostic codes are provided below:

• Diagnostic codes are assigned on a client by client basis in the Client Medical Information screen.

• Each time you Record a Session, DELPHI32 uses the diagnostic codes listed in the Client Medical Information screen as the default diagnostic codes for that session.  

• When Recording a Session, you may change the diagnostic codes for that individual session if they are different from the currently defined codes in the Client Medical Information screen.  However, since Box24e of the claim form references the codes contained in Box21, you should also modify the codes in the Client Medical Information screen.

• Box21 not only contains the actual diagnostic code, but the option to include a description of the code.  Refer to the Diagnostic Codes Setup screen to add or edit codes and their descriptions.
